Discussed BayCare's community benefit and its programs with Senator Booker's office. Promoted the idea of "food as medicine" or a form of program that Medicaid can implement that provides healthy meals for patients, as it aligns with how BayCare provides a supply of food for anyone that enters our hospitals that self designates as food insecure.
Met with leaders at CMMI to discuss better integration of patient centered models and improvement in model rollouts that allow large health systems the proper amount of time to stand up financial models so they can opt in.
Submitted comment letters for Advancing Interoperability and Improving Prior Authorization Processes Proposed Rule CMS-0057-P and 2024 Advance Notice with Proposed Payment Updates for the Medicare Advantage and Part D Prescription Drug Programs.
Met with Florida Congressional members and staff, Bilirakis, Castor, Lee, Luna, Rubio, Buchanan, Franklin to discuss behavioral health and integrating mental health services with hospital service lines to improve access. Recommended a pilot program or value-based payment model to best fund and manage the payments of these integrated services. Additionally advocated for the reauthorization of the CHGME program.
Met with Florida Congressional members and staff, as well as well as staff from CMS/CM, Bilirakis, Castor, Lee, Rubio, Buchanan, Scott, and Senate Finance Staff to discuss Hospital at Home and the inclusion of observation level patients within the current waiver model.
BayCare will continue to advocate for better payment rates in Medicare for the various proposed rules that impact payment to our doctors and hospitals.

In the comment letter submitted for the 2024 Advance Notice with Proposed Payment Updates for the Medicare Advantage and Part D Prescription Drug Programs, we advocated that there can be the potential for unintended consequences for each, for example, removal of Protein Malnutrition from the model, can jeopardize benefits such as meal services, which ultimately impacts the members that rely on these services the most.
For example, at BayCare Alliant Hospital, in 2022, managed care denials were 80% of total managed care referrals and they were only able to convert 20% of the total referrals. BayCare Alliant Hospital is one of our 16 hospitals; it is a Long-Term Acute Care hospital providing care for those with complex medical or respiratory conditions requiring an extended hospital stay.
In the Advancing Interoperability and Improving Prior Authorization Processes Proposed Rule CMS-0057-P we disputed the denials occurring and their negative impact on our patients.
Categorically, many of the denials we receive are inappropriate. Denial of claims for services already provided impacts Medicare beneficiaries of medically necessary care that they would receive if they had traditional Medicare, including access to inpatient rehabilitation facilities and Long-Term Acute Care hospital care. MA plans frequent denial of inpatient admission payment to hospitals results in a significant loss of compliant revenue to hospitals for payments for indirect medical education (IME) and disproportionate share hospital (DSH), upon which hospitals depend to continue to provide care to their communities.
